Most courses/workshops that I have taught have been evaluated using the official form that is required by Franklin Area School District. This form fits the requirements of the PA Department of Education for re-certification credit. Originally the form was paper & pencil and cumbersome to review. I digitized the form inorder to facilitate better and faster analysis. The first example is from a survey created through the district website platform, SchoolWires. The district dropped that platform in 2015 and moved to Google Sites. The form then was created using Google Forms and the results came through as a spreadsheet. There are examples of both the results spreadsheet and the summary from Google Forms.
